What is the cause of gynecomastia?

There are several reasons for developing gynecomastia. Inheritance, hormone changes, weight gain, medication are some examples. In some men, the pancreas grows unusually in puberty and it is also unusual for the breasts to grow after hormone treatment, for example in connection with disease. How does an operation go?
